REQUIREMENTS FOR ATHLETIC PHYSICALS !!
Those High School and Middle School students wishing to participate in interscholastic athletics must have an initial physical performed by their personal family physician. Due to the comprehensive nature of the new physical requirements, athletic physicals will no longer be provided at school by the Athletic Department. The PIAA has mandated the Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form as the only physical form to be accepted. These forms are available on line at the OJR Athletic homepage, the PIAA Homepage or from the Athletic office.
Below is important information concerning this form:
New Revised PIAA Physical Form dated 5/20/2010 (which will be noted on Section 2 of the form) MUST BE USED beginning June 1, 2010.
- The physical must be obtained after June 1st of each year and is good for 1 school year.
- Sections- 1, 2 and 3 must be filled out and signed by the parent and student athlete, as indicated. Be sure to sign all lines where Parent/Guardian/Student signatures are required.
- Section 4- The physician needs to fill out this portion completely - sign and date. Please make sure the physician has filled out all of the information required on the top portion of the form.(including, name, age, school, sport participating in, height, weight, pulse, etc.) and the bottom portion(including typed or printed name, address, license #, MD or DO and date)
- Section 5- This section must be completed and signed by the parent and student athlete each subsequent season during the same school year. The Athletic trainer will determine if the student athlete must be recertified by a doctor upon review.
- Section 6- If determined by the Athletic Trainer that a recertification is needed this section is to be completed by the Doctor and then returned to the Athletic Trainer.
Note: All sections of the physical form must be completed correctly prior to participation. The CIPPE form for ALL athletes must be turned into the Athletic Trainer (located in the High School) and approved PRIOR to participation.
